- Effective immediately, our former KP SCAL claims overpayment recovery/refund remittance lockbox location (File 50187, Los Angeles, CA 90074-0187) will no longer accept refund checks. Please send all refund payments to Kaiser Permanente Attn: Regional Claims Recovery, PO Box 741639, Los Angeles, CA 90074-1639.

- Please note that Self-Funded member claims may not be available in this self-service tool. If your inquiry is about a Self-Funded member claim, please sign on to the Self-Funded plans online tool.
